CardioRisk: A Self-Scored Appraisal of Your Personal Risk for Cardiovascular Disease

CardioRisk is a brief valid, and reliable questionnaire for estimating the risk of developing CVD. This appraisal may be used as an educational tool in health-promotion campaigns or as a risk measure in epidemiological studies.

CardioRisk is a 6-page folding pamphlet measuring 6" by 10". Each pamphlet contains the risk factor questionnaire, the scoring key, an example of the calculations and recommendations for risk reduction. See a sample illustration.

CardioRisk was evaluated by comparing its reliability and validity to three other self-scored risk appraisals (the Arizona Heart Test, RISKO and Determine Your Medical Age) tested in an earlier field trial.
